Do you offer consultations?

Yes. I offer a free 15 minute consultation. If there are 2 “no shows” for the scheduled appointment, the 3rd appointment will be charged a $25 consultation fee.

Can I still use my insurance if you are not a listed provider?

In these cases, I am considered an out-of-network provider for insurance plans not noted above. What does this mean? You can receive therapy services and submit reimbursement requests to your insurance provider.  I will provide a Superbill.

What is your out of pocket rate?

Intake: $175
30 min session: $75
45 min session: $150
60 min session: $200

*Sliding scale option is available.

*Superbill can be provided for out-of-network coverage for reimbursement.

Administrative documentation

$100 per form/letter requested.
This will pertain to action items such as FMLA/STD forms, treatment verification letters, etc.

How are telehealth sessions conducted?

Sessions are conducted by video on HIPAA compliant video platform. Phone sessions are conducted by providing a reliable phone number for contact. Phone sessions are not HIPAA compliant.
